Position Overview
We are seeking a caring and dependable Certified Nursing Assistant (CNA) to join our hospice team. The CNA provides hands-on support and personal care for patients, helping them maintain comfort and quality of life while also offering emotional support to families.
Key Responsibilities
Provide personal care services to patients, including assistance with bathing, dressing, grooming, and mobility.
Support patients with meals, feeding, and light household tasks as needed.
Monitor and report changes in patient condition to the nursing team.
Offer companionship, emotional support, and reassurance to patients and their families.
Maintain accurate documentation of care provided.
Work closely with the RN Case Manager and interdisciplinary team to carry out individualized care plans.
Qualifications
Current CNA certification in the state of Pennsylvania
Previous experience in hospice, home health, or long-term care preferred (but not required).
Compassionate, patient-focused approach with strong interpersonal skills.
Reliable, dependable, and able to work independently as well as part of a team.
Valid driver’s license and reliable transportation required.
